Many companies require their employees to undergo these medical evaluations at some point during their employment cycle. Our medical professionals can complete and sign the necessary forms with information about the patient’s health condition.

A school physical is a preventive exam given to children under the age of 18 that consists of a variety of categories to ensure that the child is healthy and ready to start school. This exam will give our medical professionals an idea of ​​whether your child is in good health or at risk for illness. During this exam, vital signs are taken, nutritional status, need for ophthalmology, ENT evaluation, and general physical condition are assessed. This test is usually required by a school to start the school year.

Once the exam has been performed by our licensed medical professionals, they will complete and sign the standardized Florida School Entrance Health Examination form DH3040.
